There are some standard forms we need completed in order to set up your account. We conduct a detailed review to determine whether open credit is warranted.

1. Credit Package Cover Letter
2. Credit Application
3. Bank & Trade Reference Form
4. Electronic Funds Transfer (EFT) Authorization Agreement - We require all payments to be made by automatic draft.
5. W-9 - Request for Taxpayer Identification Number and Certification
6. Last 2 years of audited company Financial Statements (Balance Sheet, Income Statement, Cash Flow Statement, and Notes)
7. Personal Guaranty Form
8. Current Personal Financial Statement
9. Tax Information
10. Authorization to Release Credit Information
